(Some) Small Print
1. You must apply in ADVANCE of your effective date. Underwriting your application could take 3 days or 3 weeks, depending on your health history. APPLY EARLY--YOU'LL BE MUCH HAPPIER.
2. Effective Dates Are:
Aetna--the 1st and 15th of the month
BCBS Georgia -- any day of the month, but no sooner than the day after you apply
Coventry -- the 1st and 15th of the month
Humana -- any day of the month, but no sooner than the day after you apply
Kaiser Permanente -- the 1st of the month
CompBenefits -- the 1st of the month; you must apply by the 15th of the month before
Assurant -- any day of the month, but no sooner than the day after you apply
3. All plans have a 12 month exclusion period for pre-existing conditions, except for Kaiser Permanente which has no pre-existing exclusion period.
The Pre-Ex exclusion means that benefits will not be paid for treatment for conditions that existed prior to the effective date of your policy. Aetna waives this exclusion if you had 12 months of previous creditable coverage and a gap between policies of no more than 63 days. If you put a condition on your application and are approved with either CoventryOne or Humana, and that condition is not excluded by a rider, it will not be excluded for Pre-Ex.
4. Acceptance is not guaranteed. Do NOT cancel your current insurance until accepted by the insurance company. No agent or broker, including us, can waive this provision.
5. Apply online via the Home page. For Aetna you can apply on line or use a paper app; most people find the paper app better. CoventryOne only uses paper applications. FAX ALL APPLICATIONS TO US AT 678.868.1892. If we do not confirm to you that we've recieved it, figure we didn't.
6. Each plan provides benefits using a network of providers. PPO and POS plans (there are no apparent differences to you between the two except that POS plans are less expensive) offer benefits, although significantly reduced, for out of network providers.
7. If maternity coverage is available and you purchase it, there is a separate 12 month waiting period and benefits are different from the other benefits of the policy.
8. Most carriers use 5-year age bands to rate premiums. so what happens when you change age bands (e.g., 20-24, 25-29, 30-34, etc.) you'll get a bigger bump than normal. Most carriers offer a first 12 month rate guarantee (BCBS Georgia does not). We do not always see your renewals so if we have not contacted you when you get a renewal, please send and email to Info@StudentInsuranceStore.com.
9. All plans run on a Calendar Year basis for deductibles, etc., except for CoventryOne which runs on a Benefit Year.
