What You Need To Apply
For Each Person Applying, Have @ Your Fingertips:
1. Name
2. A legal address for the state in which you are applying for insurance
3. An email address
4. Social Security number
5. Date of birth
6. Whether or not the person uses tobacco
7. Height and weight (even for young children or infants--seems kinda silly, but they need it)
8. The names and doseages of any medications taken in the last 5 years (sometimes 10 years) plus the name, address, and phone number of the prescribing physician. (If you have the name and an approximate address, we can often find the exact address and phone number for you)
9. A list of any hospitalizations or surgeries in the past 10 years. When they were (at least approximately), where they were, what they were for, what the treatment was, and how much you recovered. For example, John Smith was in St. Joseph's Hospital in Atlanta on 2/1/2005 for an appendicitis. His treatment was surgery, and he recovered 100%
10. A mastercard or visa for the first month's premium or your checkbook (varies by insurance carrier)
